Very nicely done, would be very handy if the scene had WASD fly around to see the smoke from all sides.

Is it possible to change the base start size? I have an object I want to be on fire, but the bottom of the smoke is too small.

Go into edit mode of the smoke particles and scale them up or down depending on the size you want them to be. Go back into object mode and now the smoke is a different size.

Can I use with Internal Render?

idk if this will help but you could set up a particle simulation and use a plane object with the texture on it.

I'm not quite sure how that would be done.
